Help with Custom Contact Page

I have run into an issue I do not know how to resolve and of course PowerSchool wouldn't help so I thought perhaps you could help out-maybe?? Please... :)

Sometime ago (12-13 SY) I installed Brent Johnson's Student Contact page from Cherryland Software site--

After our update (10.1.1) last weekend I lost that critical page. I haven't yet moved all contacts over to Jason Springel's Awesome Contact Page. The Student Contact page is there so Dr and allergies, etc. display, but the top half with the contact information is not. And the button to "ADD CONTACT" is also there, but if you click on it this is the error that I am receiving- can you give me an idea where to look.

"Alert: AJAX Error. Page=/admin/students/contacts/contactdata.html?frn=00123218&action=addcontact&sid=23268 Error=parsererror"

I did open in Blue Steel and it does indicate where it sees a problem at the beginning of the page, but I'm unsure what is actually the issue.
Any guidance would be appreciated.
