Help with a Custom Data Entry screen (accepts scanned student numbers)

I am in need of help creating a screen that creates a Truancy record for with specific information. Here's the scenario:

We send out letters to parents of students that are not picked up on time after school. We would like to create a Truancy record for each occurrence for tracking purposes. Daily, there are lots of kids that fit this bill. I would like a screen that allows a barcode scanner to be used to scan a students ID badge and create a Truancy record for each student. The reason field would be static, as well as a standard comment field to include the username or initials of the person scanning. I would also love a feature that allows entry of the student by name (in case of no ID badge) that would look up the student number needed.
