Looking for ReportWorks "Discipline by Student Report"

Hello,
I am relatively new to our district and since I have been here they have been trying to use a ReportWorks Report titled "Discipline by Student" It works intermittently and I have been tasked with trying to fix or replace it. I have done some research and I believe this may have been a report that the SIS Admin that was here before me may have gotten from a PSUG in the summer of 2011. If anyone has information about this report or who wrote it or has a copy of the actual report that could be edited or tweaked or has information on how to make this report print consistently please let me know.
