Modify Log Entry Screen

So, I get customlogentry.html. In there, it looks like it is pulling something called customlogscreen. Is there any way that we can modify the stuff within there?

I know that from the District you have the Log Entry fields which you can change around, but here is my problem. I have a field that I want to be a radio button. However, I want this radio button to allow multiple selections and there could be a minimum of 6 data pieces within that field.

Our elementary sites have what they call Rules. It sounds like all sites will be able to agree upon 6 rules, but there may be more when they get done with their committee meetings.

They want to be able to mark that a student broke rules 1, 3, and 5. If I try to make the field a radio button and add in the rules, it complains that I put in too many items and that there can only be 2. I can thieve other fields and make them radio buttons with a single selection, but this is going to put each radio button on a separate line and make it look funny.

So, any ideas on how I can make this work for my sites?
